$KAISER ALUMINUM & CHEMICAL CORPORATION SILICA PERSONAL INJURY TRUST CLAIMANT COMPENSATION OUTREACH PROGRAM The Kaiser Aluminum & Chemical Corporation Silica Personal Injury Trust (the "Kaiser Silica Trust") was established on July 6, 2006 to pay silica personal injury claims caused by exposure to Kaiser's refractory products between 1951 and 1986. The Kaiser Silica Trust is currently seeking to pay compensation to claimants who meet certain requirements. To establish a valid silica personal injury claim, several requirements must be met, which include the following: 1. The claimant has been diagnosed by a board-certified physician with one of the following silica-related diseases with prescribed exposure and latency periods: • Mixed Dust Fibrosis • Severe Silicosis • Silica-based Lung Cancer • Simple Silicosis • Complicated Silicosis The chest x-ray that was the basis for the diagnosis of silica disease must be submitted with each claim and will be reviewed by physicians hired by the Kaiser Silica Trust. 2. The claimant must have been first diagnosed with one of the silica-related diseases listed above on or after February 12, 2002. 3. The claimant was employed for at least six (6) months or more (five (5) years or more for Mixed Dust Fibrosis) between 1951 and 1986 in one of the following industries or in an alternative industry with credible evidence of actual exposure to Kaiser silica-containing refractory products: • Primary Steel and Iron Manufacturing • Aluminum Manufacturing • Glass and Ceramics Plants • Cement Plants • Ferrous and Non-Ferrous Foundries • Copper Smelting • Furnace Manufacturers and Contractors 4. In addition, a claimant who worked in one of the qualifying occupations may be entitled to enhanced recovery.
