Physician/APRN

Community Health Center, Inc. currently seeks experienced Part or Full Time primary care providers as a Family Practice Physician or Family Nurse Practitioner. Come join our team of professionals. We have oppotunities in Danbury and throughout the Central and Southeast CT region.

CHC provides comprehensive primary care services with an innovative practice model largely geared toward underserved populations, and offers opportunities for academic affiliation, teaching, multi-disciplinary practice, and a blend of individual and population based care.

MD must be from approved medical school with completion of an accredited residency program in primary care practice. Must be board-certified within 24 mo of commencing employment and hold license or be eligible for license in State of CT. APRN must be Graduate of an accredited school with MS in Nursing program with a specialty as a Family Nurse Practitioner. Maintains ANA cert as a FNP. State of CT APRN and RN licensure (within 12 months) required.
